What is Qare365?

The workload in healthcare is high for healthcare intermediaries and client service workers. For both your employees and clients, it is therefore important to set up your work processes as efficiently as possible and to automate where possible. That’s where Qare365 helps. This CRM system, specially developed by QS solutions for healthcare providers, is based on Microsoft Dynamics 365 CRM. It ensures streamlined intake management, a match between care needs and care supply, provides insight into waiting lists and supports the client journey from start to finish. Qare365 records specific care requests and key client information, and keeps track of all contact moments and follow-up actions. With smart integrations and automatic alerts, Qare365 ensures an optimal client experience and up-to-date waiting list management.

Qare365 interfaces with Fierit, mijnCaress and ONS, as well as other Electronic Client Record (ECD) systems, as standard. The system helps with client orientation, simplifies applications, intakes and matching of care offers, and makes waiting list management efficient. Integration with ONS eliminates duplicate entries and email searches, while automation of document processes saves time. Qare365 supports not only intake management, but also contract and organization management, relationship management and marketing activities, allowing you to get the best out of your organization.

 

Features

Orientation

A potential client contacts you to discuss care needs or availability. Data is captured in Qare365 and requests are processed efficiently, providing insight into the care needs within your service area.

Application

Applications come in via healthcare office, ZorgDomein, application forms or after orientation. All data is stored centrally in Qare365, making it easy to complete missing information. Qare365 helps with checks such as zip code, BSN and WLZ, optimizing data quality.

Intake

After registration, you will get to know the client better and form a complete picture of the care needs. Qare365 supports you in this in a structured and easy way.

Matching

Once care needs are clear, Qare365 searches for a suitable place. If a place is available, you nominate the client. If not, you simply put the client on the waiting list.

Screening

Some organizations require screening before a client is placed. You schedule the screening, record the results in Qare365, and can perform additional matching if necessary to continue to provide appropriate care.

Waiting list

Managing waiting lists is easy with Qare365. You have continuous insight into who, what for and how long someone is on the waiting list. With automatic emails and scheduled contact moments, you'll maintain contact with clients on the waiting list.

Site visit / lecture

When available, you plan a site visit for the client and the site to get acquainted. The result will be recorded in Qare365. If the care is not suitable, the client will remain on the waiting list or you will look for another place. If the care is appropriate, you will proceed with the placement.

Placement / administrative handling

You will proceed with placement, inform the site, arrange paperwork and ensure proper signatures. Client and care request data is forwarded to the ECD (ONS), which prevents double entry.

Stay informed

Way back when there were still floppy disks, you could file your tax return via a program on your computer. You could request a diskette with the program on it, put it in your computer, do your tax return and send the diskette back to the tax office.
